New Patient Special

• New Patients Only
• Includes comprehensive dental examination, full clean and polish, all diagnostic x-rays and photo records (if required) and fluoride treatment.
• Cannot be used with any other offer
• Patient must have valid Australian private health fund with dental cover and funds available on the day of treatment for Dental items 011 (examination), 072 (photo records), 114 (remove calculus), 121 (topical agent), 022 & 037 (x-ray & OPG)
• It is the responsibility of the patient to check their private health fund cover prior to attending the appointment.
• Must bring current, valid health insurance card on day of appointment.
• If private health fund rebate is not accepted or partially accepted via the HICAPS terminal at time of appointment, patient will be charged the non-health fund special of $265 (less any partial health fund rebate) and given an itemised receipt to present to their Health Fund to claim.
• If upon examination it is discovered that the new patient offer must be deferred until a later time the diagnosed treatment will be charged at the full rate, and the special offer checkup and clean will occur at a later date when appropriate.

Save $100 on Whitening – RDM Special

• Available for Professional Zoom! In Chair Whitening only.
• Must mention RDM special offer at time of booking.
• Offer not available on Saturdays.
• Zoom! In Chair Whitening will be charged at $590 (normal charge $690).
• Patient must attend a short consultation prior to whitening to ensure suitability for the procedure. There is no charge for the whitening consult if the whitening treatment is completed, or if the patient is deemed not suitable for the treatment. If the patient is suitable for the treatment but chooses not to proceed a $55 consult fee may be charged.

$75 Gap Only For Teens

• $75 Gap only on preventative dental appointments.
• Available for teens 13-17 years old
• Includes comprehensive dental examination, full clean and polish, all diagnostic x-rays and photo records (if required) and fluoride treatment.
• Cannot be used in conjunction with any other offer.
• Patient must have valid Australian private health fund with dental cover and funds available on the day of treatment for Dental items 011 (examination), 072 (photo records), 114 (remove calculus), 121 (topical agent), 022 & 037 (x-ray & OPG).
• It is the responsibility of the patient to check their cover prior to attending the appointment.
• Must bring current, valid private health insurance card on day of appointment.
• If private health fund rebate is not accepted or partially accepted via the HICAPS terminal at time of appointment, patient will be charged the non-health fund special of $180 less any partial private health fund rebate and given an itemised receipt to present to their private health fund to claim.
• Any additional treatment required will be charged at full rates (less any private health fund rebate paid).

Gap Free for Children

• Available for children up to the age of 12 years old
• Includes comprehensive dental examination, full clean and polish, all diagnostic x-rays and photo records (if required) and fluoride treatment.
• Cannot be used in conjunction with any other offer
• Patient must have valid Australian private health fund with dental cover and funds available on the day of treatment for Dental items 011 (examination), 072 (photo records), 114 (remove calculus), 121 (topical agent), 022 & 037 (x-ray & OPG)
• It is the responsibility of the patient to check their cover prior to attending the appointment.
• Must bring current,valid private health insurance card on day of appointment.
• If private health fund rebate is not accepted or partially accepted via the HICAPS terminal at time of appointment, patient will be charged the non-health fund special of $125 (12 years old and under) or $85 (6 years old and under) less any partial private health fund rebate and given an itemised receipt to present to their private health fund to claim.
• Any additional treatment required will be charged at full rates (less any private health fund rebate paid)

Refer a Friend

• Available only for existing patients of Dental House Gladesville referring patients who are not existing patients of Dental House Gladesville.
• Existing Patients are any patient that has attended and received treatment at Dental House Gladesville
• New Patient must relinquish their Dental House Gladesville referral card at the time of appointment with their name and the referring patients name clearly written on the card.
• On completion of the new patient’s appointment, the existing referring patient will receive a $25 Coles Myer gift card
  for the new customer referred.
• Only one $25 gift card can be claimed for each family referred.
• Gift cards will be delivered to the address on the referring patients Dental House Gladesville record, it is the referring patient’s responsibility to ensure this is correct at time of delivery of the gift card.
• There is no limit to the number of new patients (and number of gift cards received) an existing patient can refer.
• Gift cards are not exchangeable for cash.

Gap-Free Mouthguards

• Includes professionally made custom single or dual layer mouthguard (adult or child) in a single colour.  Dual colour available for an additional $15.
• Cannot be used with any other offer.
• Patient must have valid Australian private health fund with dental cover and funds available on the day of treatment for Dental items 151 (mouthguard), 071 (impressions) and 013 (consultation).
• It is the responsibility of the patient to check their private health fund cover prior to attending the appointment.
• Must bring current, valid health insurance card on day of appointment.
• If private health fund rebate is not accepted or partially accepted via the HICAPS terminal at time of appointment, patient will be charged the non-health fund special price of $150 (less any partial health fund rebate) and given an itemised receipt to present to their Health Fund to claim.
